Community Health Assessments

Ascendant Healthcare Partners is a national leader in the facilitation and development of Community Health Needs Assessments (CHNA) for state, tribal, and local health departments.

 

CHNAs provide information for problem and asset identification and policy formulation, implementation, and evaluation. Our expertise in community engagement, stakeholder engagement, data analysis, and gap prioritization make our firm unique.

Methodology

Our firm uses the new National Association of (NACCHO)’s Mobilizing for Action through Planning and Partnerships (MAPP) Evolution 2.0 ​model to conduct the community-driven planning process for community health improvement and is the ‘gold standard’ for conducting CHNAs.

Our Approach

Our individualized approach allows us to modify our methodology for each community based on the community’s unique needs. However, our core approach includes:

  • Community Partner Assessment (CPA) with stakeholders, service providers, and those involved in the community health improvement process.

  • Community Status Assessment (CSA) utilizes national, state, and local databases to provide indicators as well as a community-wide survey to tell the community’s story.

  • Community Context Assessment (CCA) which focuses on the views, insights, and values of residents within the community using a proprietary approach that builds consensus around the results and lays the foundation for action.

  • Prioritization of the community needs and service gaps in a way that builds consensus around the results and lays the foundation for action.

  • Alignment with Public Health Accreditation Board (PHAB) standards & measures.

  • Clear, well-cited, and easily deployable report and supporting materials.

  • Ongoing support and assistance, as needed.

coaching and mentoring programs for Accreditation Preparedness or MAPP 2.0.

Planning & Evaluation

Methodology

Our CHNA data to identify priority issues, develop, and implement strategies

for action and establish accountability to ensure measurable health improvement

within the Community Health Improvement Plan (CHIP), Health Equity Plan (HEP),

and Strategic Planning.

 

Our Approach

As unique as the individuals who make up your community, so are the strategies and objectives needed to make your implementation successful. Our approach includes:

  • Engagement meetings to effectively address health priorities to capitalize on core strengths and assets for poverty, social barriers, access to services, and lifestyle challenges.

  • The Social Determinates of Health (SDoH) framework and Healthy People 2030 goals and objectives are used for analysis in the development of all evaluation and planning engagements developed by Ascendant Healthcare Partners.

  • Alignment with Public Health Accreditation Board (PHAB) standards & measures.

  • Clear, well-cited, and easily deployable report and supporting materials.

  • Ongoing support and assistance, as needed.

 

Coaching

Individualized support for health department leaders to develop new skills and lead more effectively. Facilitating transition. Expert guidance in performance improvement and accreditation from a national mentor who will coach you to execute your project or program for maximum impact.

Partnership (Annual)

  • 2-full day intensive, in-person (or via Zoom)

  • 12 monthly 1-hour phone sessions

  • Full material review and a feedback session

  • Access via e-mail and text (during business hours)

  • Receive personal attention and expert guidance from a national mentor                                                                                                  who will coach you to execute your project.

Intensive Interaction (Monthly)

Public Health Accreditation Preparedness coaching will provide comprehensive technical assistance (TA) to tribal and local health departments, such as:

  • Documentation Review, Recommendations, and Development

  • Accreditation Road-mapping
